Forms

The information you obtain from the interview forms is meant to be integrated into your psycho-educational reports. The counseling forms section comprise the (a) Release of Information forms, these forms are used when wanting to access mental-health information from outside agencies/professionals, and (b) the Non-DIS Counseling forms when working with Non-DIS students on a temporary/short term basis. A selection of consultation forms are available, depending upon the specific needs of your case.

Logs and Payroll Documents

Here are the required absence certification forms (illness and non-illness) in addition to the monthly payroll forms and monthly federal activit logs. These forms will be submitted with your payroll for each month. However, please note that the federal activity logs you submit will reflect work that was completed for the previous month.

Human Relations, Diversity and Equity Department – Website includes information and resources on creating a positive school/work climate, diversity, anti-bullying resources and conflict resolution.
School Report Card – Website to access School Report Cards. Provides community with information related to how well a school is meeting district goals towards graduation, proficiency for all students, 100% attendance, school safety, and parent, student, and community engagement.
Special Education Positive Behavior Support – District website for information related to Positive Behavior Intervention and Support (PBIS).
LAUSD Discipline Foundation Policy – Provides resources for developing school wide behavior expectations, behavior charts, behavior reflections, and behavior contracts. Includes videos and links to outside resources.
